This is the Payment form for In-Person Fees and Banquet Fees.
In-Person Attendance = 80 USD
Banquet Fee Per Person = 116 USD
Online Attendance = No Fees
Please fill out and submit the form below and an invoice will automatically be generated. A separate email with payment on how to pay electronically will be emailed to you if you choose one of the electronic method of payments.
Since we need to notify the facility ahead of time how many people will attend the banquet, the deadline for cancellation is Monday, December 5th.
We strongly prefer that you pay electronically. Electronic payments will be processed via VENMO, PAYPAL and ZELLE. Once you have completed your payment, your ticket(s) and confirmation will be sent via email. If you have paid electronically and cancel by Monday, Dec. 5th, you will receive a full refund.
We will also accept cash payment on-site at the event, for those who are unable to pay electronically. However, please realize that if you choose this option we are relying on you to provide the funds, unless you cancel by Monday, Dec. 5th.
